Incident Report for Material Without Student Liability

This form must be used when, during the course of a laboratory session, an incident occurs that causes the breakage, damage, or loss of materials, and the student is not responsible for it. The instructor must describe the incident, specify the ongoing practice, and either take responsibility or confirm that the student followed all instructions correctly.

The purpose of this report is to exempt the student from paying for or replacing the material, and it must be submitted to the Coordination of Chemistry Laboratories and Storage at the following email address: laboratoriosquim.fcqi@uabc.edu.mx
